Altitude is a rate problem long before it is a height problem
The body adjusts to thin air over days rather than hours, which is why how fast you got there matters more than the number on the signboard.

Thin air is a pressure problem, not a shortage of oxygen
Air at four thousand metres contains the same proportion of oxygen as air at sea level. What has changed is the pressure of the whole column above you, and with it the partial pressure of the oxygen in each breath. Less pressure means less oxygen crossing from the lung into the blood with every inhalation, even though the mix itself is unchanged.
That distinction matters because it explains why breathing harder helps only up to a point. You can move more air, and the body does exactly that within minutes of arriving high, but you cannot raise the pressure driving oxygen across the lung membrane. The rest of the adjustment has to happen elsewhere and it happens slowly.
It also explains why altitude is not a single threshold. Effects begin to appear for some people not far above two thousand five hundred metres and become common much higher, but the variation between individuals is wide and does not track fitness, age or previous experience in the way people assume. A strong walker is not protected.
Acclimatisation is a sequence, and the useful parts are not instant
The first responses come quickly. Breathing rate rises, the heart works harder, and the kidneys begin adjusting the blood chemistry that the changed breathing has disturbed. Those are hours-to-days processes and they carry most of the early load.
The changes that actually make high altitude comfortable — more red cells, better delivery of oxygen into working tissue — take considerably longer, running over weeks rather than days. Nobody on a two-week trip completes that process. What a traveller is realistically doing is the early adjustment, and giving it enough time is the whole game.
This is why the standard mountain advice is about pace rather than about limits. Gaining height slowly, sleeping lower than the highest point reached during the day, and building in a rest day before going higher are all ways of buying the body time. None of them is a guarantee, and none of them is a substitute for descending when someone is genuinely unwell.
The Leh problem is entirely a rate problem
Leh sits high enough that arriving there is a significant altitude gain for almost anyone, and there are two completely different ways of doing it. The road from Manali or from Srinagar climbs over several days, crossing high ground and dropping again, with nights spent at intermediate heights. The flight takes an hour from the plains.
Physiologically those are not the same journey at all. Flying in delivers you to a substantial altitude with no adjustment whatsoever, which is precisely why the local convention is to spend the first day or two doing very little. That advice is not caution for its own sake; it is the only acclimatisation the flight route allows.
The road has the opposite trade. It is slower, harder and dependent on the passes being open, but the ascent profile is much kinder. People who drive up and fly out often have an easier time than those who do it the other way round, which is worth knowing before you book what looks like the more convenient pair of tickets.
Sleeping height is the number that counts
Itineraries in the Indian Himalaya frequently cross passes far higher than anywhere the traveller will sleep, and the two heights have quite different significance. A couple of hours at a very high pass, with a descent afterwards, is a much smaller physiological event than a night spent at a moderately high village.
That is why experienced planners talk about the sleeping altitude of each night and about how much it increases from one night to the next, rather than about the highest point on the map. Climb high, sleep low is an old piece of mountaineering practice and it is the reason a route can include a spectacular pass without punishing everyone on it.
The corollary is that a route which raises the sleeping altitude sharply, several nights running, is demanding regardless of how easy the driving is. Sitting in a vehicle does not exempt anyone. Some of the least comfortable altitude experiences in India happen to people who have not walked a step.
What to do with all of this
Treat the above as a description of a mechanism, not as medical advice. Anyone with a heart or lung condition, anyone pregnant, and anyone taking regular medication should have this conversation with a doctor before booking a high-altitude trip, because the interaction between an individual condition and low pressure is not something a travel article can assess.
The general practices that mountain communities and medical guidance broadly agree on are unglamorous: ascend gradually, take a rest day when you have gained significant height, drink enough, go easy on alcohol for the first days, and treat worsening symptoms as a reason to go down rather than to push on. Descent is the one intervention that reliably works.
Practically, this shapes an itinerary more than most people expect. A rest day at the start is not a wasted day. It is the day that makes the following six possible, and a plan with no slack in it is a plan that will make the decision for you at the worst moment.
Common questions
Does being fit help?
Not in the way people hope. Fitness makes the physical effort of a mountain trip easier, but susceptibility to altitude does not track it — strong athletes are affected and unfit travellers sometimes are not. Fitness may even encourage people to ascend faster, which is the one thing that reliably raises the risk.
Is the drive to Leh safer than the flight?
The ascent profile is gentler, which is a genuine advantage. Against that, the road crosses very high ground, is only open in season, and is a long journey in itself. Neither option is risk-free; they simply carry different risks, and the road buys you acclimatisation that the flight does not.
How long should the first rest day be?
There is no universal number, and any specific figure you read should be treated as a rule of thumb rather than a rule. The common practice after flying into a high town is to keep the first day or two very light and to avoid gaining further height until you feel normal at the altitude you are at.
